Child Drop Off & Pick Up Authorization

As parent/guardian, I hereby authorize the following persons to drop off/pick up my child from Fairview Baptist Church for Next Level. I hereby agree to inform the following persons that proper identification may be required in order to drop off/pick up my child.

Waiver and Conditions of Enrollment

I/we, the parent or legal guardians, undertake and agree to indemnify and hold completely blameless Fairview Baptist Church of Lindsay, its pastors, board of elders, staff and/or volunteers from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of Fairview, as well as any medical treatment authorized by the supervising individuals representing the church. I/we authorize the Fairview church staff or ministry volunteer staff to sign consent for medical treatment, and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant named above in the event that the parent/guardian or provided emergency contact cannot be immediately reached. This consent and authorization is effective only when participating in and/or traveling for events of Fairview’s ministries. I/we also grant permission for the reasonable use of pictures and/or videos of the student taken during ministry time for promotional purposes (i.e. website, church program promotions, Fairview’s social media, etc). The camp pastor/director reserves the right to dismiss a camper who, in the pastor/director’s opinion, is a hazard to the safety and the rights of others, or who appears to have rejected the reasonable rules of camp.

Next Level Registration Fee

There is a $30 registration fee per child, and money will be received on the first day of camp. Please indicate which method of payment you will use. If you need financial assistance to ensure your child may attend, please contact Pastor Mike Bodkin at mike@fairviewc.com.
